"Brains" still has a solid non-slang meaning. We also sell internationally and I can think of a lot of words or jokes we wouldn't touch, including ones based on words that are 'ok' in American English but not elsewhere. I'm not posting them here ;). There was actually a urban clothing company that once used the slogan " go to college and get more brains" the company had to remove the ads.So when I saw the t-shirt you posted sorry to say I had to giggle a little.
